That includes yours.You will report to the Associate Medical Director at our South Daytona Clinic.Why You'll Love Working HereTeam-Based Care Model: Partner with a dedicated care team that supports patients' physical, emotional, and social wellness.More Time with Patients: See fewer patients per day and spend more time delivering meaningful care.Supportive Culture: We foster a welcoming, inclusive environment where teamwork and growth are prioritized.Your Role & ResponsibilitiesProvide comprehensive care to seniors following clinical standards.Collaborate daily with your care team to ensure coordinated, high-quality care.Manage referrals, hospital/SNF coordination, DME, and home health services.Document care with support from quality-based coders.Participate in on-call rotation and support growth plans at your center.Work on-site.What We're Looking ForRequired:Master's degree in nursing with board certification, or graduation from an accredited PA program with board certification.Active, unrestricted NP or PA license in the state of Florida.At least one year of experience in value-based care, primary care, or Medicare Advantage settings.Active DEA license and Medicare/Medicaid provider numbers, or the ability to obtain them.Familiarity with HEDIS quality indicators and Medicare guidelines.Scheduled Weekly Hours: 40Pay Range: $123,800 - $170,300 per yearThis job is eligible for a bonus incentive plan.
