Elderwood is seeking a dedicated full-time SLP to join our rehabilitation team. Located at 225 Bennett Road, our 172-bed skilled nursing and rehabilitation community offers long-term care, subacute rehab, and specialized memory care services in a warm, supportive environment. What You'll Do: Evaluate residents and develop individualized speech-language therapy treatment plans. Provide therapy for swallowing disorders, communication deficits, and cognitive-linguistic impairments. Conduct audiometric screenings as requested and recommend further audiology services when appropriate. Collaborate with PTs, OTs, nursing staff, and physicians to optimize resident outcomes. Educate residents and families on progress and treatment strategies. Maintain accurate documentation and ensure compliance with all regulatory standards. What We're Looking For: Master's Degree in Speech-Language Pathology. Current New York State SLP license. Certificate of Clinical Competence (CCC‑SLP) preferred. Skilled Nursing Facility (SNF) or geriatric experience is a plus. Strong communication and teamwork skills. Passion for helping residents regain function and quality of life. Responsibilities Screens resident for communication problems upon request; conducts an in-depth evaluation and makes written recommendations for a therapy plan of care. Coordinates audiometric screening test for resident and makes referrals for further audiology evaluations or treatments if necessary. Initiates speech-language therapy plan of care according to physician orders; maintains required contact with the physician during the therapy program of resident. Observes resident rights and treats residents with respect and kindness. Maintains written evaluations, progress notes, and discharge summaries for all residents in a speech-language treatment program. Confers with other staff regarding special needs of residents and integrates speech-language treatment program with goals and approaches of other disciplines. Attends Resident Care Planning and Family/Resident Care Plan Review meetings for residents currently in a speech-language treatment program. Maintains records including statistical reports about resident, in-service education information, budget information and a material/equipment inventory. Submits statistical reports to the Business Office staff for Medicare billing payments, and completes assessment forms upon request. Attends staff meetings and required or preferred in-service training. Participates in developing and conducting in-service training programs for facility personnel involved in resident care. Reviews written speech-language policies and procedures annually and makes necessary revisions. Maintains costs of program within the parameters of the annual budget. Qualifications M.A. or M.S. Degree in Speech-Language Pathology from an approved school. State license to practice as a Speech-Language Pathologist. Clinical Competence Certification from the American Speech and Hearing Association preferred. Ability to communicate well verbally and in writing. Current, valid CPR/BLS credential. Qualities of maturity, diplomacy, and ability to work well with others. Verification of qualifications by the State Department of Health. Rehabilitation Therapies license required. Ability to perform tasks to established standards of excellence. Demonstrated willingness to uphold the vision and mission of the company following core values. Regular interaction with residents, coworkers, visitors, and/or supervisors, requiring communication in English at an intermediate or higher level.
