Nurse Practitioner

Job Summary BRC Mental Health Services/Boise Residential Care is hiring a full time Nurse Practitioner to work with adults in an outpatient clinical setting to provide medication management, nursing and overall care to clients living in our Residential Treatment Housing. We are looking for a Nurse Practitioner to join us in creating a new type of housing and mental health clinical services all in one program for those dealing with serious and persistent mental illness (SPMI) or both SPMI and substance abuse. We need someone who is passionate about generating creative ideas to help clients maintain stable environments and transition into the community. You can work the days and times that work best for you ranging between 5-15 hours per week, but we are willing to pay more for working Mondays or Wednesdays during business hours. Needs to be self-motivated and be able to work independently and identify times when clients need assessed and followed up with. Applicant must have reliable transportation to get to and from the houses and agency. Can work as an independent contractor or employee. Also needs to have own computer and Internet access as well as a cell phone so staff is able to contact the NP when not at the agency or houses. Computer knowledge is a must as all documentation (treatment plans, notes, assessments, etc.) is done in our web based computer program. Will also be able to choose what days and times you want to be on call. On call duties include responding to phone calls, text messages or emails regarding medications, resident's conditions and incidents and give recommendations for staff to follow. On call is just being able to take phone calls. You will not need to go into the agency or houses when get calls. You will be able to set appointments up yourself. Responsibilities and Duties Prescribe, and/or direct psychotherapeutic treatments or medication to treat mental, emotional, or behavioral disorders in coordination with the BRC and the State treatment teams Performs on-going evaluation of severe and persistent mentally ill and their psychosocial behavioral patterns on a regular basis either through an audit of progress notes, medications, and behavior management on a regular basis Conducts health assessments at admission, quarterly, Incident Reports or when needed Consults with other clinical staff and other specialists concerning specific medical problems of clients Provides face-to-face, phone intervention, one-on-one at the agency or house for evaluation, treatment, recommendations on a regular basis and during psychiatric emergencies Demonstrates effective time management and organization skills within the office setting Assesses client in their home and or at the agency. Provides mental health education to client, family, agency staff and housing staff. Documents all mental healthcare contacts. Delegates to all staff with necessary protocol delegations. Abides by the policies and procedures of the agency as well as each group home that the agency is contracted with. Collaborate with physicians, clinicians, agency coordinated care staff, nurses, housing staff or other professionals to discuss service plans, treatment planning and progress either face-to face, phone and/or in the computer program in their progress notes or care notes. Participate in the development of individualized Treatment Plans, using a variety of treatments and attend individual Treatment Planning for each client that occurs on a regular basis and in emergencies Review and evaluate treatment procedures and outcomes of other coordinated care, housing staff and medical professionals Review progress notes and med changes to make sure all follow up is completed Proper documentation of all billable services will be required to be completed within 24 hours Participate in any in-service training and supervision for Team members Must be self-motivated, hard-working, independent Completion of forms, reports or progress notes in a timely manner. Collaborate with all members of the treatment team in regards to the clients' strengths and recovery goals. Maintain a nonjudgmental approach, use effective listening, eye contact, professionalism, and positive interactions at all times. Respect the rights and dignity and the privacy and confidentiality of those they serve.
