Key Responsibilities Evaluate patient condition and functional level Develop and implement personalized treatment plans Supervise PTAs and HHAs per plan of care Provide in-home therapy and adaptive equipment guidance Maintain accurate and timely documentation Educate patients and families on home exercise programs Submit notes and visit documentation promptly Adhere to HIPAA confidentiality standards Qualifications Active PT license in Ohio 1+ year of PT experience (preferred) OASIS and WellSky/Kinser experience (strongly preferred) Current TB test and CPR certification Valid driver’s license and auto insurance Ability to attend orientation in Dublin, OH #J-18808-Ljbffr Nightingale Home Healthcare Of Ohio Inc
