Behavioral Health Care ManagerLifekind Health is looking for a full-time Behavioral Health Care Manager to serve as a critical link between behavioral health, medical services, and care coordination teams. This role supports compliance with Enhanced Care Management (ECM) program requirements while improving patient engagement, continuity of care, and clinical outcomes for moderate to high-risk populations. Help to increase show rates, ensure compliance, and optimize clinical staff efficiency by shifting non-billable tasks away from licensed providers.Our mission is to bring care that's whole, human, and healing. Our team of medical doctors, psychologists, chiropractors, acupuncturists, and dietitians work together within a revolutionary transdisciplinary model that addresses the quadruple aim of healthcare: enhancing patient experience, improving patient health, reducing healthcare costs, and increasing employee satisfaction.Schedule: Monday – Friday, 8:00 am – 5:00 pmLocation: Works out of 2 clinics in the region: Rancho Mirage and IndioAbout the position:Provide Enhanced Care Management (ECM)-aligned care coordination for high-acuity patients and patients with comorbid behavioral and medical conditionsConduct proactive patient outreach, engagement, and follow-upIdentify and address barriers to care (e.g., transportation, psychosocial factors)Monitor behavioral health status and escalate clinical concerns appropriatelyFacilitate communication across interdisciplinary care teamsEnsure accurate, timely, and compliant documentation per payer requirementsSupport continuity of care across behavioral health, medical, and ancillary servicesOur Ideal Candidate will have the following qualifications:Master's degree in Social Work (MSW) or Marriage and Family Therapy (MFT) (unlicensed); or Master's in a related mental health field with an Associate Professional Clinical Counselor (APCC), Associate Clinical Social Worker (ASW), or Associate Marriage & Family Therapist (AMFT) certificationBasic Life Support (BLS) or CPR certification requiredExperience in behavioral health, care coordination, or case management strongly preferredStrong interpersonal and communication skillsAbility to work within a multidisciplinary care modelKnowledge of Medi-Cal and ECM program requirements preferredAbility and willingness to travel to the 2 clinics in your assigned regionAt Lifekind, we believe our employees deserve the best.
