Community Outreach & Strategic Partnerships Manager Healthy At Home Caregivers

Orange County, California | Full-Time | Field-Based $60,000–$74,000 Annually + Performance Bonus Company Vehicle Provided

Healthy At Home Caregivers is seeking an experienced Community Outreach & Strategic Partnerships Manager to lead business development, expand referral partnerships, and increase access to home care services throughout Orange County and surrounding Southern California communities.This is not a traditional sales position.We are looking for someone who understands California's community-based healthcare system, enjoys building meaningful professional relationships, and is passionate about connecting seniors and vulnerable adults with services that allow them to remain safely and independently in their homes.Success in this role comes from becoming a trusted community resource, educating referral partners, and creating long-term partnerships that improve access to care.

Key Responsibilities Develop Strategic Referral Partnerships

Build and strengthen professional relationships with:

• Hospitals and health systems
• Discharge planners, case managers, and social workers
• Physicians, medical groups, and specialty clinics
• Skilled nursing and rehabilitation facilities
• Assisted living and independent living communities
• Home health and hospice agencies
• Veterans Affairs programs and community partners
• Area Agency on Aging and senior centers
• Alzheimer's and dementia organizations
• Adult Day Health Centers
• Community clinics and Federally Qualified Health Centers (FQHCs)
• Latino Health Access and other community-based organizations
• Faith-based organizations
• Managed care organizations and CalAIM Community Supports providers
• Community resource agencies serving older adults, individuals with disabilities, and underserved populations

Community Engagement

Represent Healthy At Home Caregivers at networking events, healthcare meetings, educational programs, senior expos, caregiver workshops, and community outreach events.Educate healthcare professionals, community organizations, seniors, and families about available home care services, family caregiver opportunities, Veterans benefits, dementia resources, and community-based care programs.Develop new partnership opportunities throughout Orange County and surrounding Southern California communities.

Business Development

Maintain a strong referral pipeline and document outreach activities using AxisCare or a comparable CRM.Work collaboratively with leadership to identify growth opportunities, evaluate market trends, and implement strategic outreach initiatives.Promote Healthy At Home Caregivers as a trusted provider of compassionate, high-quality home care services. 

Requirements:

 

 

Required Qualifications

• Minimum 3–5 years of successful business development, outreach, referral development, or account management experience within the home care industry
• Direct home care outreach experience is required. Applicants without home care business development experience will not be considered.
• Strong working knowledge of California's CalAIM program and the community-based referral network that supports eligible members.
• Experience building relationships with organizations participating in or supporting CalAIM, including managed care organizations, Enhanced Care Management (ECM) providers, Community Supports providers, community clinics, senior service organizations, and other community-based partners.
• Knowledge of the Medicare GUIDE Model, Veterans Affairs Community Care, and community resources supporting individuals living with dementia and their caregivers is highly desirable.
• Thorough understanding of referral development within the Orange County healthcare and senior services community.
• Excellent networking, communication, presentation, and relationship-building skills.
• Self-motivated with strong organizational, planning, and follow-up abilities.
• Professional appearance with exceptional customer service skills.
• Valid California driver's license with a clean driving record and willingness to travel throughout the service area.

Preferred Qualifications

• Existing professional relationships within Orange County's healthcare or senior services community.
• Experience using AxisCare or another healthcare CRM.
• Bilingual English/Spanish.
• Experience conducting educational presentations, community workshops, or professional training sessions.

Compensation & Benefits

Annual Salary: $60,000–$74,000, based on experience

Performance Bonus
