Oregon Licensed MTM Pharmacist

To support a dynamic Clinical Engagement Services Center, the contract-to-potential-FTE Oregon Licensed MTM Pharmacist will perform telephonic consultations, conduct comprehensive medication reviews, and provide patient education while working remotely with flexible hours. Key responsibilities: Engage members telephonically to schedule consultations and provide clinical recommendations based on medication therapy management protocols Ensure patient privacy and confidentiality by adhering to HIPAA regulations and organizational policies Document patient care notes and recommendations accurately and participate in departmental meetings to enhance clinical programs Required qualifications: Current and unrestricted Pharmacist license in the state of residence Strong knowledge of disease state management guidelines and the 5 Core Elements of MTM Services 1+ years of experience working remotely within a cross-functional team environment Intermediate proficiency with Microsoft Word, Outlook, and Excel Minimum education of a Bachelor's Degree or Pharm D
