What You'll Do:

• Assess, plan, and implement individualized rehabilitation programs for patients with physical functioning disorders and wound care needs.
• Utilize evidence-based clinical practices and standardized assessments to support optimal patient outcomes.
• Perform procedures including manual techniques, therapeutic exercises, ambulation, modalities, wound care, and support device management.
• Monitor patient progress, adjust treatments, and evaluate outcomes to ensure effective care delivery.
• Provide education, counseling, and collaboration with patients, families, and healthcare teams.
• Document all clinical activities accurately in electronic records, adhering to regulatory standards.
• Support supervision of rehab aides, assistants, and students as required.
• Participate in team meetings, trainings, and ongoing professional development.

Required Skills:

• Valid PT license in Massachusetts.
• Strong knowledge of rehabilitation techniques, wound management, and patient assessment.
• Excellent communication skills and ability to work collaboratively within a multidisciplinary team.
• Proficiency in electronic documentation systems.
• Ability to interpret and apply clinical regulations and standards.

Nice to Have Skills:

• Experience in long-term care or skilled nursing facilities.
• Familiarity with DME procurement and supply management.
• Bilingual abilities are a plus for diverse patient populations.

Preferred Education and Experience:

• Bachelor's or Doctorate degree in Physical Therapy.
• Prior experience in LTC or similar healthcare settings.
• Licensure and certified in accordance with state and national standards.

Other Requirements:

• Must be able to work day shifts in Worcester, MA.
• Ability to obtain necessary DME, supplies, and certifications.
• Commitment to adhering to APTA and state practice acts, ethics, and professional standards.
• Willingness to participate in training and team activities.
