Collaborate with multidisciplinary teams to improve community health outcomes.Job DescriptionPrimary Care Physician OverviewServe as a Primary Care Physician, delivering comprehensive and personalized healthcare services to diverse patient populations.Focus on preventive care and holistic health improvement within a collaborative healthcare environment.Utilize advanced medical technologies to enhance patient care and documentation processes.Engage in professional development opportunities to refine clinical skills and advance career growth.Contribute to initiatives aimed at improving healthcare accessibility and patient outcomes.Maintain manageable patient panel sizes to ensure focused and personalized care delivery.Collaborate with multidisciplinary teams to deliver high-quality, coordinated care.Join a mission-driven organization dedicated to community health improvement and innovation.Primary Care Physician Key Responsibilities & DutiesDiagnose and manage acute and chronic health conditions, creating tailored care plans for patients.Collaborate with interdisciplinary teams to ensure comprehensive and coordinated patient care.Provide health education and coaching to promote preventive care and healthy lifestyles.Coordinate with specialists and other providers for seamless transitions in patient care.Ensure accurate and timely documentation of clinical encounters and patient progress.Participate in quality improvement initiatives to enhance healthcare delivery standards.Adhere to regulatory standards and organizational policies for ethical healthcare delivery.Engage in community outreach activities to expand healthcare access and patient recruitment.Primary Care Physician Job RequirementsDoctor of Medicine (MD) or Doctor of Osteopathy (DO) degree is required.Board certification in Internal Medicine, Family Medicine, or Geriatrics is mandatory.Active state medical license or eligibility to obtain one promptly is essential.Basic Life Support (BLS) certification required within 90 days of employment.Proficiency in electronic medical records and healthcare technologies is preferred.Strong commitment to serving underserved communities and improving health outcomes.Excellent communication and leadership skills to effectively manage and inspire care teams.Ability to travel locally for patient care and occasionally for training or recruitment.Fluency in English; additional language skills may be advantageous.
