Services include intake, referral, outpatient therapy, psychiatric assessment, service coordination, substance abuse treatment, training and social rehabilitative services, crisis intervention, partial hospitalization,community outreach,dayand recreational services for adults with intellectual disabilities, homelessoutreachand housing program. As a Mental HealthResidentialCoordinator with Chartiers Center in our Residential TreatmentProgramyouareresponsible forprovidingassistancetoadults facing mental health challenges. Job duties include: Develop, implement, monitor, and document Residential Service Plans for an assigned caseload of residents. Completing necessary skill training in the areas of daily living through hand over handassistance, direct verbal and written presentation of information and observation, as assigned. Accurately complete all assignedobservation,intervention, documentation of resident self-administration of medication, medication counts and necessary follow-upin accordance withProgram policies and procedures. Accurately implement recommendations of the treatment team to ensure consistency within the program. Maintain resident records in accordancetopolicies,proceduresand regulations. Complete laundry,cleaningand shopping asrequired. Effectively respond to,reportand document all emergencies/crises/incidents in accordancetorelevant policies,proceduresand regulations. Assistthe Director of the Residential Treatment Program to effectively oversee the daily operations of the program and inscheduling ofstaff /overall management of the schedule.
