Options for Community Living, Inc. is seeking a Medical Case Manager to join our team on Long Island. The role focuses on coordinating medical care for residents in our community residence program, including those with complex health needs. An ideal candidate has an associates degree (or 60 college credits) in health, human services or education, or is an LPN with two years of qualifying experience.
