Transplant Coordinator III-NP Under the direction of a collaborating physician, the Transplant Coordinator III‑NP assists in the care of patients with a variety of end‑stage organ diseases in inpatient and/or outpatient settings. Key Responsibilities Coordinate and participate in the medical and surgical management of transplant and end‑stage organ disease patients. Perform comprehensive history and physical examinations, develop differential diagnoses, implement the plan of care, and document patient responses. Order medications, diagnostic studies, and therapeutic procedures and track results in a timely manner as outlined. Assist in the care of patients under the direction of a collaborating physician and support other personnel to ensure high‑quality delivery of patient care. Develop and coordinate an organized system for patient flow from initial referral, through evaluation and education, to post‑transplant follow‑up and referral back to the community. Partake in collaborative interdisciplinary research activities, process improvement, and evidence‑based practice initiatives. Attend professional meetings and conferences, maintain professional affiliations, and pursue professional development to remain current with advances in transplant populations and end‑stage organ diseases. Manage alongside a supervising physician post‑transplant patients for the Las Vegas transplant clinic, and engage outreach to referring cardiologists in Las Vegas to boost referrals. Participate in a 24‑hour on‑call rotation, relaying information to physician staff and facilitating ongoing patient care, including handling organ offers. Perform additional special projects and duties as assigned. Required Qualifications Master’s degree or Doctorate degree in Nursing Practice Nurse Practitioner Program. Two years’ experience as an Acute Care Nurse Practitioner focused in Cardiology, Advanced Heart Failure, and/or Cardiothoracic Surgery. Five years of clinical nursing experience, including at least three years in transplant, cardiothoracic ICU, or Cardiology. Two years’ experience as a Nurse Practitioner working in transplant, cardiothoracic surgery, or cardiology. Excellent communication skills; ability to speak, read, and write English proficiently. Knowledge of medical procedures and conditions; ability to understand, follow, and perform duties, including whole blood collection and apheresis procedures, in compliance with policies and regulations. Preferred Qualifications At least two years of transplant coordination experience with specific expertise: heart (cardiology), lung (pulmonary), liver (abdominal transplant), and kidney/pancreas (abdominal transplant). At least one year of Nurse Practitioner experience is preferred. At least two years of managing heart transplant patients. Required Licenses & Certifications Registered Nurse – RN (CA Board of Registered Nursing). Acute Care Nurse Practitioner – NP (CA DCA), board certified as an Acute Care NP if the practice setting is inpatient. Nurse Practitioner Furnisher (CA DCA). DEA certificate. National Provider Identifier (NPI). Basic Life Support (BLS) – healthcare provider credential from the American Heart Association. Fire Life Safety Training (LA City); if no card upon hire, one must be obtained within 30 days and renewed before expiration. Certification as a Nurse Practitioner through the American Association of Nurse Practitioners or American Nurse Credentialing Center. Nurse Practitioner – Nevada State Board; if no license upon hire, must be obtained within three months of hire or transfer date.
