RN Navigator

Kaiser Permanente is seeking an RN Navigator to guide patients through complex care pathways in our Healthcare & Medical Services department. The RN Navigator will coordinate appointments, educate patients and families, and collaborate with multidisciplinary teams to optimize outcomes and satisfaction. Responsibilities include care planning, tracking diagnostic and treatment milestones, supporting care transitions, and removing barriers to care. This role requires strong clinical judgment, excellent communication, and proficiency with electronic health records to ensure seamless, patient-centered navigation across the continuum of care.
