MDS Registered Nurse (RN) - PRN

The MDS Nurse is responsible for organizing, completing, and evaluating resident assessments and care plans to promote optimal resident functioning, ensuring compliance with state and federal regulations. Key Responsibilities Coordinate MDS, CAAS, and care planning processes upon resident admission. Conduct monthly triple checks for Medicare, managed care, and outpatient therapies. Observe, report, and document changes in residents’ physical and emotional conditions. Establish care plan dates within 7 days of assessments or reviews. Ensure nursing documentation aligns with care plans during treatments. Communicate assessment reference dates (ARD) with the interdisciplinary team. Interview residents and families for accurate assessments, and consult with physicians as needed. Participate in quality improvement programs and community inspections. Report and investigate resident abuse, neglect, or property misappropriation. Perform administrative duties including documentation and reporting. Mentor and support professional growth of other MDS nurses. Respond promptly to crises or assistance requests. Qualifications & Skills Licensed Practical Nurse (LPN) or Registered Nurse (RN) with a current license. At least 2 years of MDS experience; skilled nursing background preferred. Strong communication skills, proactive approach, and ability to anticipate needs. Ability to foster a professional, respectful, and caring environment. #J-18808-Ljbffr
