Activities Assistant Reports to : Activity Director Job Qualifications: High School Diploma or equivalent necessary Individualized skills in areas such as crafts, music, education or physical activities helpful Friendly, outgoing personality, with a warm, caring and patient attitude Willingness to work as part of a team Essential Job Functions: Assists in planning and implementation of organized program for recreation Provides a planned resident‑specific, goal‑directed recreation program for assigned residents/patients; facilitates small group activities as assigned by the director Ensures that all charted recreation progress notes are informative and descriptive of the services provided and indicate the resident/patient’s response to the service; assists in developing a written comprehensive plan of care for each resident/patient recreation program that identifies the problems/needs and the goals to be accomplished; follows the established plan of care in providing the daily recreation program Involves the resident/patient and family members in the assessment and care planning process when possible Transport residents/patients to and from recreation programs Job Types: Part‑time, Per diem Benefits: 401(k) 401(k) matching AD&D insurance Dental insurance Disability insurance Health insurance Life insurance Paid time off Referral program Tuition reimbursement Vision insurance Medical specialties: Geriatrics Shift: Day shift Ability to commute/relocate: Marlborough, MA 01752: Reliably commute or plan to relocate before starting work (Required) Experience: Skilled Nursing Facility: 1 year (Preferred) #J-18808-Ljbffr Alliance Health and Human Services
