Locum - Physician - Hematology Oncology Longmont, CO

Mon-Thursday - 7a-5p with 30 minute unpaid lunch.

Ongoing dates will be as needed for vacations coverage; typically 2-3 weeks per quarter.

Reason for Coverage: Vacation submitted by current locum provider

EMR Required Training:

Module (learn lessons) Must be completed prior to virtual EMR training to attend the training.

EMR Virtual training Only able to attend if Modules are completed before training. Virtual training must be completed prior to the providers first day.

Additional Details / Questions Answered by Facility:

Census: Patient population, Adult, Geriatric;

Admissions, Growing. Currently about 4 clinic patients per day.

Submission Requirements

Board Certified Medical Oncology - Required

Active CO license - Highly preferred, must be able to license before start date.

Active BLS - Required for credentialing

Active DEA - Required for credentialing (site will consider a transferred DEA, however, must keep this DEA for the duration of the assignment, not just for shifts worked.)

Candidate must be comfortable and willing to see Hematology patients - Required - Must note at name clear

Must acknowledge candidate understands the EMR training requirements set forth in job description and will complete in this order to avoid missing their start date - Required

Candidate must be available for the stint detailed in job description to be considered - Required

Clean MP/background - Required for quick credentialing, site will place priority for candidates with a self inquiry NPDB at time of presentation.
