Job Duties:

• Collects data from patient, family or significant others and records on Initial Data Base form.
• Contributes to the formulation and updating of plan of care and implements plan of care under the direction of the Registered Nurse.
• Gives direct nursing care to assigned patients (e.g., medications, treatments, dressing changes, TPR) according to established standards.
• Recognizes changes in patient condition and responds appropriately, notifying charge nurse as needed.
• Documents pertinent information regarding patient's responses and nursing interventions in patient's medical record.
• Administers medication according to hospital policies.

Patient Care Related Duties:

• Applies principles of infection control and safety in the performance of patient care.
• Communicates effectively with charge nurse and other member of the Health Care Team regarding patient's needs and/or changes in condition.
• Responds appropriately in crisis situation.
• Reports significant unit problems to the R.N.
• Customer Service: respect, flexibility, knowledge, confidence, professionalism, pleasant attitude, patience and helpfulness. All responses should be timely, professional, caring, and respectful in accordance with Customer Service Performance expectations.
• Maintains established departmental policies and procedures, objectives, quality assurance program, safety, environmental, and infection control standards.
• Assess gaps in policies and procedures, and create necessary policies and procedures to fulfill these gaps.
• Understands and adheres to the Medical Center's Code of Conduct. Familiar with the Medical Center's Mission, Vision, and Values Statements.

Other Job Duties:

• Orients patient and family to new environment; develops rapport with patients family and/or significant others.
• Orients LPNs, Nurses' Aides and orderlies as required.
• Effectively utilizes the hospital computer system as it pertains to job responsibilities and in accordance with hospital policy.
• Participates on unit committees and in CQI activities as assigned.
• Participates in patient care conferences and in staff development activities as required.
• Assists other personnel as needed. Performs other related duties as required.

Education/Licensure:

• Graduation from an accredited school of Practical Nursing or equivalent education as described by the State Education Law.
• Current NJ license as an LPN.
• Successful completion of Progressive Care Orientation Program required.

Skills:

• American Heart Association Basic Life Support certification required.
• Clinical nursing skills appropriate for an LPN.
• Good oral and written communication skills.
• Good interpersonal skills.
• Speaks, reads and writes English to the extent required by the position.
• Job Setting/Physical Demands: All patient care areas; contact with patients, visitors, staff. Frequent prolonged standing/walking. Lifts, positions, pushes, and/or transfers patients. Protective clothing worn: face shield, gloves, impermeable gown, and particulate respirator.
• Salary commensurate with experience within posted range.

Based on experience $33.22-$39.42 per hr differentials eve $1.60 night $1.70 weekend $1.00
