- NV license (or willingness to obtain) - REQUIRED
- Clean Malpractice/License History - HIGHLY PREFERRED
- Most recent NPDB Self Query Report (within 30 days active) - REQUIRED no exemption, required at presentation
- Does the provider require visa support - J1 or H1B? (Note in "Additional Submission Details" field) - REQUIRED
- DEA (must be obtained prior to start) - (note in "Additional Submission Details" field) REQUIRED no exemptions
- CSR/CDS (must be obtained prior to start) - (note in "Additional Submission Details" field) REQUIRED no exemptions
- Current APRN national specialty certification (Neonatal Nurse Practitioner - NNP-BC)
- Master's degree or higher in Nursing
- Certification and licensure as a Neonatal Nurse Practitioner
