Project Coordinator

California-Health-Collaborative-

Since 1982, the California Health Collaborative (CHC) has a long history of working with youth and their families through culturally responsive and community-centered public health programming. With a mission of “changing lives by improving health and wellness", CHC is committed to addressing and advocating for health justice among California’s underserved and underrepresented communities who have limited access to resources, and often face barriers related to culture, language, income, race, education, gender, geography, or immigration status. JOB SUMMARY: Through funding from the California Department of Public Health – Tobacco Prevention Program, the California Health Collaborative is focused on reducing health disparities among and engaging the Asian American Native Hawaiian Pacific Islander (AANHPI) populations through a healing-centered tobacco prevention program. Asian Pacific Islanders Partners and Advocates Countering Tobacco (API PACT) is a Central Valley-based tobacco control program serving Merced and Fresno County AANHPI communities with intentional community engagement that centers voice, agency, wellness, culture, and healing as we build momentum towards ending California’s tobacco epidemic. The California Health Collaborative is recruiting one full-time Project Coordinator (PC) for the API PACT Project. This position will be based in either Livingston or Clovis, CA. With support from the Director of Program Services, the Project Coordinator will lead the team in the engagement of AANHPI community partners and advocates of all ages in program activities for API PACT to implement smoke-free outdoor areas and multi-unit housing (MUH) policies. ESSENTIAL FUNCTIONS: Consults with the Project Director relative to planning, policy, and program direction. Responsible for being the lead community engagement staff member in creating safe and inclusive environments for participants, including youth, engaged in API PACT. Presents and models values and behavior consistent with inclusivity, respect, and affirmation of all partner identities. With support from the Project Director, develops program progress reports, correspondences, promotional and other program materials. In coordination with the Community Engagement Coordinator (CEC), assists in the oversight and planning for all program activities, including strategic planning sessions, policy meetings, community events, evaluation activities and educational presentations. Facilitate the coordination of either a youth, young adult or adult coalition in partnership with Livingston and Clovis leaders to invite various sectors and agencies in both communities to get involved in program initiatives. Support the CEC in the coordination of the Community Engagement Agreements (CEA) Program to engage local AANHPI serving organizations in program efforts. Support the CEC with development and coordination of paid media and social media advertisements and maintaining the program’s social media pages to share educational information, cessation resources, and program highlights and updates. Lead the coordination, development and maintenance of a program website that will act as a central hub for networking, strategizing, and information dissemination. Supports in reducing barriers for youth participation by identifying methods to meet youth where they are at, identifying modes of transportation to support youth in accessing program activities, and on occasion transports youth in personal vehicles. Be available to meet with youth and adult program participants during the school day, afterschool or on weekends or evenings as required to meet the needs of the community. Ensure community advocates are central to the planning of all program activities and ensure authentic engagement in the project’s efforts in Livingston and Clovis. Supports the CEC in the development of coalitions tasked with leading Policy, Systems and Environmental change in Clovis and Livingston regarding a smoke-free outdoor area and/or MUH policy. Take a lead role in supporting the Community Engagement Coordinators in developing ongoing trainings for partners and volunteers on topics related to: Policy and Systems Change, Telling your Personal Story, Stigma-Free Tobacco Prevention, Healing Centered Engagement, Working with Elected Officials, and Advocacy. Support the Project Director in training and leading the efforts of the Community Engagement Coordinator to ensure project activities are moving forward, team members feel supported and are successfully demonstrating contract compliance. Develop community partnerships to support the goals of the API PACT program, including establishing relationships with school campuses, among elected officials and community organizations. In coordination with the Project Director, provides support with fiscal oversight for the program including overseeing the program budget and assisting in completing budget modifications. Under the supervision of the Project Director, assists with the oversight of personnel to the program, including training of and supporting program staff in achieving performance goals and completing ongoing evaluation activities and support in preparing biannual reports of the impacts of the project. Represents the California Health Collaborative at meetings with committees, community agencies, care providers and related entities, and facilitates inter-agency linkages, communication, and collaboration. Performs other duties as assigned. OTHER JOB REQUIREMENTS Attendance at off‑site program activities throughout the communities of Livingston and Clovis multiple times a month is required; Attendance at various mandatory out of town/overnight trainings is required. QUALIFICATION REQUIREMENTS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. REQUIRED EDUCTION AND/OR EXPERIENCE: Education and experience requirements are a bachelor’s degree in Public Health, Community Health, Health Administration, Social Work or a related field and a minimum of two years’ experience working with diverse communities; one year leadership experience required; OR A combination of work experience, community partnerships, and education indicating an ability to fulfill the requirements of the position. One year of community experience related to community organizing, youth engagement, and community outreach may be substituted for each year of college credit. LANGUAGE SKILLS: Ability to follow verbal and written instructions accurately. Ability to document and summarize project activities in clear, concise written formats. Ability to share updates and observations during team meetings to support program improvement. Ability to communicate effectively with program staff, participants, parents, and community partners in culturally diverse settings, conveying information in a clear and accessible manner. REASONING ABILITY: Demonstrates sound judgment in planning and facilitating curriculum sessions and project activities, with the ability to thoughtfully respond to values-based questions from participants. Applies problem-solving skills to address practical challenges within established guidelines and program expectations. Supports the creation of safe, inclusive, and affirming spaces for all youth by modeling and reinforcing best practices in positive youth development. Uses critical thinking to adapt approaches based on participant needs and evolving community conditions, including identifying when program guidelines may need adjustment. Prioritizes resources effectively and objectively to support successful implementation of the project scope of work. Recognizes when to seek guidance and collaborate with the Project Director and program staff to ensure informed and appropriate decision-making. Cross-Program Collaboration Collaborate across CHC programs to increase awareness of CHC services, promote CHC-branded programs and standardized referral pathways, and consistently utilize CHC’s online referral asset (including the CHW Navigation hub and related support programs) to strengthen access, coordination, and continuity of care. External Awareness & Representation Proactively promote CHC priorities, programs, and referral assets to all external stakeholders—including enrollees, partner organizations, and community entities—to eliminate fragmentation, prevent missed referral opportunities, and ensure consistent awareness and utilization of CHC services. Professional Standards and Virtual Meeting Expectations Employees are expected to maintain a professional and business-appropriate appearance and conduct during all work-related interactions, including virtual meetings. The employee is required to have their camera turned on and remain visibly present during all company business meetings conducted through Zoom or other approved virtual meeting platforms unless otherwise authorized by management due to exceptional circumstances.
