Behavioral Health Case Manager II

Delaware State Jobs

Behavioral Health Case Manager IICoordinates behavioral health services for individuals with mental health and/or substance use disorders through assessment, care coordination, utilization review, case management, referral, and transition planning. Collaborates with hospitals, providers, community agencies, and internal staff to ensure timely access to appropriate services while maintaining accurate documentation and compliance with program requirements.
