Job Description & Requirements Specialty: Case Management Discipline: RN Duration: 13 weeks 40 hours per week Shift: 8 hours, days Employment Type: Travel RN Home Health Case Manager Schedule Monday to Friday 8:00 AM – 5:00 PM 8-hour shifts No on-call requirement Counties covered: Orange Durham Alamance Compensation & Benefits - Mileage reimbursement: $0.48 per mile- Guaranteed 40 hours/week Required License - Active, unrestricted Registered Nurse (RN) license- Compact RN license accepted Required Experience & Skills Home Health care experience (mandatory) Experience with HCHB (Homecare Homebase) system OASIS documentation experience (required) Med-Surg and/or critical care experience preferred Strong clinical assessment and decision-making skills Ability to manage high-risk patient populations Education - Associate Degree in Nursing (ADN) or- Bachelor of Science in Nursing (BSN) or related field Performance Expectation - Approx. 6 patient visits per day About Care Career Care Career brings together a portfolio of leading healthcare staffing organizations, each delivering specialized talent solutions across the industry.
