In this position you will play a critical role in caring for older adults at the end of life - not only physically, but emotionally, socially, mentally, and spiritually. The Admission Nurse facilitates and manages all aspect of the hospice admission process including appropriateness for admission, develops an individualized plan of care, related to the patients' needs, in collaboration with the physician, patient, and their families/caregivers. Determines the appropriate utilization of resources based on clinical assessment, regulations and established clinical best practices.Essential tasks and duties include providing ongoing assessments of physical, psychological, social, and spiritual needs of the patient and family, provide education, supervision and counseling to the client and their family/caregiver regarding care needs as appropriate to client's needs, providing accurate and appropriate documentation of patient/family services.QualificationsStrong desire to serve older adults with a passion for end-of-life care.Registered nurse with current licensure with the State Board of Nursing in state(s) in which he/she practices.A minimum of three years of nursing experience preferred.Critical care/oncology, long-term care, or medical/surgical background with strong knowledge of pain and symptom management preferred.Hospice/Home Care experience is a plus, but not required.Excellent interpersonal skills.Ability to work with dying patients and caregivers in the home, hospital, and other facility settings.Ability to work as a member of an Interdisciplinary Team.Strong assessment skills, experience with symptom management preferredStrong oral and written communications skills.Computer skills helpful.Reliable transportation, current driver's license and current auto insurance.Demonstrated compatibility with PHS's mission and operating philosophies.Demonstrated ability to read, write, speak and understand the English language to communicate with all customers.Pay RangeThe estimated starting pay range for this role is $82,813 - $108,000 per year. This good faith estimate is based upon PHS's typical starting pay range for this role.
