What You'll Do Manage an assigned hospice caseload while serving as the primary nurse for patients and families. Complete comprehensive nursing assessments and develop individualized plans of care. Deliver expert pain and symptom management focused on comfort, dignity, and quality of life. Educate patients, caregivers, and families throughout every stage of the hospice journey. Coordinate care with physicians, nurse practitioners, social workers, chaplains, hospice aides, therapists, and other interdisciplinary team members. Provide care in private homes, assisted living communities, and skilled nursing facilities. Maintain accurate, timely EMR documentation while ensuring compliance with Medicare hospice regulations. What You'll Bring Active Minnesota Registered Nurse (RN) license. Experience in hospice, oncology, ICU, palliative care, home health, or another high-acuity clinical setting. Strong assessment skills, sound clinical judgment, and the ability to work independently. Passion for relationship-centered, patient-focused care. Excellent communication, organization, and critical thinking skills. Reliable transportation and willingness to travel throughout the service area.
