Admission LPN, $1,500 Sign-on-Bonus M - F 8am - 4:30pm Fort Pierce, FL, US Salary: $25.00 - $26.00 per hour The LPN Admissions Care Coordinator primary role will be responsible for reviewing newly admitted hospice patient’s charts for accuracy and will QA all necessary forms needed for admissions. He/she will follow‑up with patients on the NTUC list & provide additional information about hospice services. Responsible for answering incoming telephone calls from patients/caregivers/medical professionals. Ability to react quickly and calmly, exercise good judgement and make sound decisions during urgent and routine communications. Proficient in receiving, obtaining and relaying accurate information to be disseminated. Provides skilled supportive patient and family care over the telephone in accordance to certification restrictions. May be required to visit patients when needed. Provides care within scope of practice. Qualifications Current CPR certification. Minimum of three years of nursing experience in a home health/hospice setting within the past five years. Preferred knowledge of hospice, Medicare, Medicaid and private insurance benefits. Must be able to handle sensitive issues appropriately using sound judgement, maintain professional boundaries, and maintain confidentiality. Must be self‑directed and able to work independently or as a team. Basic computer skills, use of common office software and EMR software experience. Excellent people and relationship-building skills. Excellent customer service skills. Maintain effective work relationships with co‑workers, the public and administration. Highly organized, adaptable problem solver and ability to work under stress or pressure. Attendance is a condition of employment. Ability to work a variety of shift hours, to include days, nights, weekends and holidays. Be current in legal and compliance hospice issues. Responsible for upholding Treasure Coast Hospice/Model Standards for patient visits and compliant documentation. Provide skilled professional nursing care to the patient and family unit under RN supervision, incorporating physical, psychosocial, spiritual, cultural, and biological components. Utilize the nursing process, demonstrate accurate data collection, and assist in planning, implementation, and delivery of clinical care services. Implement the nursing plan of care in accordance with physician orders and under the RN Case Manager. Provide appropriate pain and symptom management, including patient responses to treatment and medications. Document patient care reflecting nursing intervention, patient response, and progress towards goals. Maintain clerical records via computer or handwritten data. Provide effective, supportive, and sensitive approach to patients, families, and staff, always polite and courteous. Develop program strategies for Treasure Coast Hospice, supporting leadership, compliance, and strategic plan success. Teach and uphold hospice standards with focus on documentation, visits, communication, and professional conduct. Lead, develop, and motivate employees with confidence, energy, integrity, and resources. Provide verbal assistance over the telephone and care to patients and families. Actively support the compliance program, reporting as required, and complying with all federal, state, and local laws.
