Hospice Admissions LPN: Care Coordination Expert

Treasure Health is looking for an Admission LPN to coordinate hospice admissions and provide patient care both over the phone and in-person. The role involves reviewing charts for accuracy, responding to calls, and ensuring high-quality patient interactions.
