RN Case Manager - Home Health - Full Time, DaysJoin our dedicated team as an RN Case Manager in Home Health, where you will make a significant impact on patient care. You will work with a cohesive patient-care team, allowing you to develop meaningful relationships with patients while maintaining a balance between professional responsibilities and personal life.ResponsibilitiesScreen, evaluate, re-evaluate, and treat patients.Develop care plans in collaboration with physicians, patients, and caregivers.Periodically review and recertify patient care plans as appropriate.Coordinate care plans through interdisciplinary communication and case conferences.Provide ongoing assessments of clinical, functional, and emotional patient status.Document evaluations, treatment goals, and objectives accurately.Teach self-care techniques to patients and caregivers as needed.Provide medication, diet, and other instructions as ordered by physicians.Required Experience / Certifications / LicensureMust be a licensed Registered Nurse in Vermont and New Hampshire or hold an unencumbered compact license.Graduate of an accredited school of nursing.2 years of experience in Home Health required.ScheduleFull-time, DaysLocationVarious locations including Bradford, VT; Randolph, VT; Springfield, VT; White River Junction, VT; and Lebanon, NH.Why ARMStaffing?At ARMStaffing, we take care of our employees!
