RN Case Manager – Home HealthSchedule: Full Time, DaysAssignment Details: Holiday coverage required.Requirements:Must be a licensed Registered Nurse in the state or hold an unencumbered compact license.Graduate of an accredited school of nursing.2 years of experience in Home Health.BLS/BCLS certification required.CPI, ENPC, RNFA, TNCC, AWHONN, STABLE, ASLS, NBSTSA, APHON, CMA, CCMA, RMA, C-EFM certifications are preferred.Responsibilities:Develop and periodically review patient care plans in collaboration with physicians, patients, and caregivers.Coordinate patient care through interdisciplinary communication and case conferences.Conduct ongoing assessments of patients' clinical, functional, and emotional status.Document evaluations, treatment goals, and objectives.Teach self-care techniques to patients and caregivers, including medication and diet instructions.Client Details: State VTJob Board Disclaimer: TLC Nursing Associates, Inc. is an equal-opportunity employer and fully complies with all applicable federal, state, and local laws.
