Job Summary CVHS is seeking a full‑time Behavioral Health Care Coordinator with a Bachelor’s degree in Social Work or related field to provide behavioral health care coordination, case management, and resource support services within an integrated primary care setting. Services will be provided to a diverse patient population with complex behavioral health and social needs. Key Responsibilities Assist with outgoing referrals for higher levels of care Conduct pre‑behavioral health diagnostic screening visits Address social determinants of health and assist patients with applications for disability and other social service resources Network with community agencies, maintain regional resource relationships, and collaborate closely with behavioral health clinicians and medical providers as part of a multidisciplinary care team Travel to assigned clinics as needed to meet with patients, network within the community, attend in‑person meetings, and participate in occasional trainings Document all services in the electronic medical record in a timely manner Provide mental health first aid for patients in crisis Maintain guidance, reference documents for BH PSR’s on SharePoint Train BH PSR’s in managing BHS schedules, referrals and insurance verification Develop and maintain resource directory of services that are potentially useful for our patient population Explore resource options as unique needs arise Maintain data related to the entire behavioral health department as directed Participate in CVHS organizational and quality improvement meetings representing Behavioral Health Assist BH Director with creation of departmental reports Promote integrated behavioral health services within and outside of CVHS Knowledge, Skills, and Abilities Excellent customer service skills and abilities Knowledge of, sensitivity to, and compassion for persons experiencing behavioral health crises Ability to triage BH patients and assess client BH clinical needs Willingness to assist patients with concrete needs and comfort with technology for documentation, data management, and promotion to include use of the electronic medical record, data management software, and word processing software Work collaboratively with a multidisciplinary team Willingness to participate in organizational quality improvement initiatives Promote BH services such as representing BH at health fairs and community meetings Education, Training, and Experience Bachelor or Master Degree in social work or a human service field Experience serving patients in a public behavioral health or medical setting Mental Health first aid certification (can be obtained after employment) Desired Qualifications Spanish‑speaking preferred but not required Other Information CVHS is primarily a remote/work‑from‑home position but expects occasional in‑person work at assigned clinics.
