Requirements Doctor of Medicine or Doctor of Osteopathic Medicine Degree Previous work experience as a physician for at least 5 years preferred Adept at diagnosing problems and creating treatment plans Has basic knowledge of regulatory compliance and reimbursement with Rural Health Clinics and Critical Access Hospitals Experience working as a hospitalist Current license to practice in the State of Colorado Completion of residency in family practice or board certified by either the Board of Family Practice or the Advisory Board of Osteopathic Specialty Current ACLS, BLS, PALS, TNCC, ENPC Certification Current DEA and DPS registration Familiarity with electronic medical record systems Excels in communicating sensitive and/or adverse information Sound judgment and expected decision-making ability Analytical thinker and complex problem-solving skills Bilingual in English/Spanish is a plus Summary The RHC Medical Provider/Hospitalist is responsible for providing comprehensive, high-quality patient care across both the Rural Health Clinic (RHC) and hospital settings. The provider delivers primary and family medicine services, including preventive care, health maintenance, acute and chronic disease management, and addressing patients' social and behavioral health needs. The provider evaluates, diagnoses, treats, and manages patients while coordinating appropriate testing, referrals, medications, and follow-up care. When serving as the hospitalist, the provider rounds on and manages hospitalized patients, including admissions, daily assessment and treatment, coordination of care, discharge planning, and transitions of care. The provider works collaboratively with nursing, ancillary departments, medical staff, and outside healthcare professionals to ensure continuity, quality, and safety of patient care. As a provider in a rural healthcare setting, the position is expected to develop strong relationships with patients and actively participate in community service. Ideally, the provider will reside in or near Lake County and become an engaged member of the community. Rural Health Clinic / Primary Care Duties Provides comprehensive primary and family medicine services to patients of all ages. Performs preventive examinations, annual wellness visits, routine physicals, and health maintenance services. Evaluates and treats acute illnesses, injuries, and other episodic healthcare needs. Diagnoses, treats, and monitors chronic medical conditions and develops appropriate long-term care plans. Obtains and documents complete medical histories and performs appropriate physical examinations. Orders, reviews, and interprets laboratory tests, diagnostic imaging, and other clinical studies. Prescribes medications, treatments, therapies, and other interventions within the provider's scope of practice. Provides patient and family education regarding diagnoses, treatment plans, medications, preventive care, and follow-up recommendations. Identifies social, behavioral, and environmental factors that may impact patient health and coordinates appropriate resources or referrals. Makes appropriate referrals to specialists and coordinates follow-up care. Provides continuity of care for established patients and assists with appropriate follow-up after hospitalization, emergency department visits, or specialty care. Completes medical records, orders, referrals, results review, patient communications, and other required documentation in a timely manner. Participates in daily clinic huddles and works collaboratively with nursing, medical assistants, Patient Access, and other members of the care team. Supports clinic quality measures, preventive health initiatives, patient access goals, and other RHC performance expectations. Hospitalist Duties Provides hospitalist coverage according to the established provider schedule. Evaluates and admits patients requiring inpatient, observation, or other hospital-level care within the capabilities of the facility. Performs admission history and physical examinations and establishes appropriate plans of care. Rounds on hospitalized patients and provides ongoing assessment, diagnosis, treatment, and medical management. Places and manages appropriate inpatient orders, medications, diagnostic testing, and treatment plans. Reviews laboratory, imaging, and other diagnostic results and adjusts treatment plans as clinically appropriate. Responds to changes in patient condition and determines when additional intervention or a higher level of care is necessary. Coordinates transfers to higher levels of care when patient needs exceed the facility's capabilities. Communicates with receiving providers and facilities to support safe and timely patient transfers. Collaborates with nursing, pharmacy, laboratory, imaging, rehabilitation, respiratory therapy, case management, and other departments involved in patient care. Participates in multidisciplinary care planning and discharge planning. Completes discharge summaries, medication reconciliation, follow-up instructions, and other required discharge documentation. Coordinates appropriate follow-up with the RHC, specialists, home health, rehabilitation, or other healthcare services following discharge. Provides appropriate handoff communication to other providers during hospitalist coverage transitions. Supports Swing Bed patients and interdisciplinary care planning when applicable and within the provider's scope and assigned responsibilities. Clinical Leadership & Organizational Duties Utilizes appropriate leadership skills in directing, delegating, coordinating, and evaluating patient care practices. Maintains effective communication and collaborative working relationships with medical staff, nursing, clinical departments, administration, and outside healthcare professionals. Participates in provider meetings, staff meetings, daily huddles, training, and other organizational meetings as required. Participates in quality improvement, patient safety, peer review, utilization review, and other clinical initiatives as assigned. Supports compliance with applicable federal and state regulations, Critical Access Hospital and Rural Health Clinic requirements, organizational policies, and medical staff standards. Maintains accurate, timely, and complete clinical documentation to support continuity of care, quality reporting, and appropriate billing. Maintains patient confidentiality and complies with HIPAA and organizational privacy and security requirements. Maintains required professional licensure, credentialing, privileging, certifications, continuing education, and competencies. Promotes health education and preventive care in collaboration with other healthcare professionals. Participates in community outreach, health education, and other activities that promote the health and well-being of the community. Develops positive relationships with patients, families, community members, and local healthcare partners. Demonstrates professionalism, ethical conduct, sound clinical judgment, and patient-centered decision-making. Works collaboratively with the Clinic Manager, Medical Director, and hospital leadership to address clinical and operational needs. Completes duties and responsibilities assigned by the Clinic Manager and Medical Director that are consistent with the provider's role and scope of practice.
