RN Case Manager: Discharge & Care Coordination

Inova Health System is seeking a dedicated Registered Nurse Case Manager 3 to join the Case Management team in Falls Church, Virginia. This role is crucial in developing and implementing patient care plans, ensuring timely management throughout the continuum of care. The ideal candidate will work closely with physicians and nursing staff to provide effective discharge planning and coordination of services. The position requires an experienced professional with at least 4 years in case management and a Bachelor's degree in Nursing. Certification in Basic Life Support is also necessary.
