Physical Therapist – Outpatient Rehab Employment Status: Full time | Shift: Day | Location: 1935 W Main St, Salem, VA Position Overview The Physical Therapist confirms and interprets physician referrals, performs patient assessments, and plans and implements individualized treatment programs tailored to each patient’s developmental or chronological age and medical diagnoses. The role includes supervising Physical Therapist Assistants (PTAs) and collaborating with a multidisciplinary care team. Sign‑on bonus eligible. Responsibilities Confirm and interpret physician referrals. Perform patient assessments and develop individualized treatment plans. Implement and monitor treatment programs to achieve patient‑defined goals. Coordinate patient care and clinical activities with patients, families, therapists, and other health‑care team members through written and verbal communication. Document patient information in the medical record and complete related forms in compliance with regulatory agencies and institutional policies. Supervise PTAs and collaborate with therapy staff, support staff, volunteers, and students to ensure efficient clinical activity and quality patient care. Participate in departmental activities, including in‑services, performance improvement, program development, new employee orientation, committee membership, peer review, and staff meetings. Deliver high‑quality customer service and patient care, demonstrating respect, dignity, kindness, and empathy in every encounter. Champion the organization’s mission, vision, and values and comply with all policies, procedures, ethical standards, and the code of conduct. Provide problem‑solving, consultation, and interpretation services to colleagues and external vendors as required. Suggest cost‑containment initiatives within the department or service line. Ensure compliance with state licensure and federal regulations when supervising PTAs. Qualifications Graduate of an approved school of Physical Therapy. Education: Bachelor’s degree in Physical Therapy and minimum 5 years of experience, **or** Master’s/Doctorate degree required. Experience in the specific level of care preferred, but not required; one year of clinical experience required for Home Health positions. Licensed to practice in the Commonwealth of Virginia by the Virginia Board of Physical Therapy. American Heart Association Basic Life Support (BLS) certified within six months of employment (Carilion approved program). Effective interpersonal, communication, customer service, and teamwork skills.
