Physical Therapist - Outpatient Rehab Full time. Carilion Outpatient Therapy. Sign on bonus eligible. Certified Lymphedema Therapist a plus. The Physical Therapist (PT) confirms and interprets physician referrals, performs patient assessments, and designs individualized treatment plans tailored to the patient’s developmental and chronological age and medical diagnoses, in accordance with state and professional regulations. Provides patient care based on physician referral and age‑appropriate assessment, aiming to achieve goals set with patient or family. Coordinates patient care and clinical activities through communication with patients, families, therapists, clinical leaders, managers, physicians and other health care team members. Documents in the patient's medical record and completes other patient‑care forms in compliance with regulatory agencies and hospital policies. Supervises PTAs and collaborates with therapy staff, support staff, volunteers and students to achieve efficient clinical activity and quality patient care. Participates in departmental and professional activities as requested/assigned, including presenting in‑services, performance improvement projects, program development and evaluation, new employee orientation, committee membership, peer review and staff meetings. Delivers high‑quality customer service and patient care, demonstrating respect, dignity, kindness, and empathy in every interaction with patients, families and coworkers. Champions Carilion’s mission, vision, and values; complies with policies, procedures, ethical standards and code of conduct. May work at other Carilion facilities to meet staffing needs and may provide home‑care visits using personal transportation. Work requires physical activity with patients, including lifting patients who may weigh over 200 lbs. Candidates must also be knowledgeable about life‑span growth, development and departmental policies. What We Require: Education: Graduate of an approved school of Physical Therapy. Bachelor’s degree with 5 years of experience, or Master’s or Doctorate degree required. Experience: Site‑specific level of care preferred but not required. Home health positions require at least one year of clinical experience. Licensure/Certification: Licensed to practice in Virginia by the Virginia Board of Physical Therapy. Life Support: AHA BLS‑certified within six months of employment. Other: Effective interpersonal, communication, customer service and teamwork skills required.
