Provide hands‑on care, administer medications, and evaluate patient progress. Maintain and submit timely documentation, including OASIS assessments. Collaborate with therapists, caregivers, and healthcare professionals to ensure comprehensive care. Educate patients and families on treatment plans and self‑care techniques. Inform patients of additional services and resources as needed.
