POSITION DESCRIPTION

JOB TITLE:Prevention& ReferralSpecialist

REPORTS TO:Prevention Services Manager

POSITION OVERVIEW:

ThePrevention& ReferralSpecialistis responsible forproviding individualswithhealth information, referrals, testingand treatment, risk reduction interventions,condom distribution, and awareness.Position also follows up on referrals toassistindividuals with overcomingbarriersand ensure linkage to services.

RESPONSIBILITIES AND DUTIES:

• Providesgroup-levelfacilitation.

• PerformsLinkage to Care activities.

• ProvidesHIV/HCV counseling and testing Services at Center and infield locations.

• ConductsQuality Assurance, including, but not limited to, daily opening of testing labs and documentation of temperatures

• Provides service navigation.

• Tracks referrals and linkages

• Provides partnerservices/disclosureassistance.

• Providesrisk and harm reduction counseling

• Maintains organization of risk/harm reduction materials.

• Performs office/Centerduties as needed.

• Collects client-level and aggregate data.

REQUIRED:

• A valid drivers license and reliable transportation isrequired
  

EDUCATION AND EXPERIENCE:

• High School Diploma or equivalent

• A minimum of 1 year of experience inrelatedfield

CLASSIFICATION:Full -time/Non-Exempt
