Home Care Coordinator

The Home Care Coordinator is responsible for managing all aspects of the staffing process and managing the scheduling of shifts, to include scheduling open shifts. This position also requires establishing and maintaining positive relationships with all related entities (internal staff, clinical field employees, contract staff, patients/families, referral sources, etc ), responding to staff requests/concerns and maintaining information to assist in the coordination of patient services and staff availability.

Pay Range: $23 to $25 hourly

Duties & Responsibilities

1. Triage/manage incoming phone calls for Namaste Home Care locations

2. Match/schedule all new clients to caregivers quickly and efficiently.

3. Review and address any open shifts daily.

4. Data entry for new clients ensuring compliance and accurate detail. Upload admission paperwork to the EMR.

5. Coordinate/collaborate with the marketing team, report out on how many shifts are available and in what areas.

6. Prepare reports for scheduling accountability meetings. Attends, and actively engages in scheduling and clinical accountability meetings.

7. Cross-train as a caregiver to support call outs and urgent caregiving needs.

8. Support with supervisory visits, orientation, and other caregiver training as needed.

9. Prepare admission packets, resource packets, orientation binders, and other administrative assistant responsibilities.

10. Become familiar with home care payors and actively engage with growth strategies.

11. Maintain information on current staff and contract staff demographic information, skill sets, availability, and productivity levels.

12. Assist in the day-to-day operation of the Namaste Home Care offices and ensure start-of-care packets are prepared for use.

13. Accurately complete all workflow associated with staff requests for scheduling changes, caseload reassignments, recertification of services, new referral scheduling, missed visit, visit time exceptions, visit time change requests, and all associated workflow as assigned.

14. Communicate effectively, professionally, and thoroughly with staff regarding coordination of care expectations; educate and enforce deadlines; and establish and maintain positive working relationships with current staff, contract staff, community partners and others.

16. Demonstrate commitment, professional growth, and competency.

17. Maintain comprehensive working knowledge of contractual staff relationships.

18. Serve as an administrative support resource for all clinical and marketing staff.

19. Ensure compliance with all state, federal, and Medicare regulatory requirements.

20. Participate in the on-call rotation.
