Physical Therapist - Rehabilitation

The Physical Therapist will screen, evaluate, and rehabilitate patients with physical functioning disorders and wound care, developing appropriate care plans in accordance with regulatory and clinical practice standards. Utilizing standardized assessments and evidence-based practices, the therapist will support clinical interventions and administer various rehabilitation procedures, including manual techniques, ambulation, therapeutic exercises, and modalities.

Responsibilities

• Assess and evaluate patients' progress and modify treatments accordingly.
• Provide comprehensive discharge summaries in line with regulatory requirements.
• Document all necessary information in the electronic system and maintain patient records.
• Communicate effectively with supervisors and health team members regarding patient care.
• Consult and counsel patients, families, and caregivers about physical disorders and rehabilitation options.
• Supervise and support physical therapist assistants and rehabilitation aides/students.
• Participate in required meetings, trainings, and conferences.

Schedule

This position requires an 8-hour day shift, 5 days a week, with no on-call or weekend duties.

Required Experience / Certifications / Licensure

• 1 year of Physical Therapy experience preferred; new graduates with Level II experience are welcome to apply.
• Experience in long-term care (LTC) or rehabilitation settings is required.
• Current Physical Therapist license in Oklahoma is mandatory.
