The organization has four acute care hospitals and one specialty hospital that together house over 1,800 hospital beds. *Position Information:* We have a team of 5 physician SNFists and 5 APP's as well as looking to grow to provide services for 3 post-acute rehabilitation facilities. That in itself is an incredible patient to provider ratio and an example of Lee Health's commitment to provide a Quality Based care approach! As a SNFist, you will dedicate your expertise to post-acute care environments. This role will allow you to address the specific needs of patients transitioning from hospital care to rehabilitation or long-term residency in SNFs (Skilled Nursing Facility) and SNU's Skilled Nursing Unit). You will create a pivotal shift towards specialized, quality care tailored for a vulnerable segment of the population often in dire need of attentive, personalized medical oversight.
