RN Case Manager - Hospice

About the Role:

We are seeking a highly skilled RN Case Manager RNCM Hospice to join our team in the Health Care Services industry. As an RN Case Manager RNCM Hospice, you will be responsible for providing end-of-life care to patients in hospice care. You will work closely with physicians, nurses, and other healthcare professionals to ensure that patients receive the highest quality of care. Your primary goal will be to manage patient care and ensure that patients receive the appropriate level of care based on their individual needs. You will also be responsible for coordinating care with other healthcare providers and ensuring that patients receive the necessary support and resources to manage their symptoms and improve their quality of life.

Minimum Qualifications:

• Active RN license in the state of California
• Excellent communication and interpersonal skills
• Ability to work independently and as part of a team
• Strong organizational and time management skills

Responsibilities:

• Provide end-of-life care to patients in hospice care
• Manage patient care and ensure that patients receive the appropriate level of care based on their individual needs
• Coordinate care with other healthcare providers
• Ensure that patients receive the necessary support and resources to manage their symptoms and improve their quality of life
• Maintain accurate and up-to-date patient records

As an RN Case Manager RNCM Hospice, you will use your excellent communication and interpersonal skills to build relationships with patients and their families. You will also use your strong organizational and time management skills to manage patient care and ensure that patients receive the appropriate level of care based on their individual needs. Your ability to work independently and as part of a team will be essential in coordinating care with other healthcare providers.
