Growing together.Primary Responsibilities:Completes and documents psychosocial assessments, chemical dependence consults, intervention and coordination of care to identified populationsProvides crisis intervention/counseling to identified populations to enhance patient family coping mechanisms regarding illness and adaptation for families to their illnessEngages the patient family/significant other and interdisciplinary team in the discharge process to identify appropriate post acute resources and community service follow-upsProvides education to patients/family/significant other and interdisciplinary team in regard to psychosocial factors related to illness and ongoing support of needsActively participates in those areas of expertise to further enhance the role of social work within the hospitalCompletes duties as required for the purpose of discharge planning, i.e. options forms, agency referrals etc.Complies with standards and requirements in medical/legal reporting situationsComplies with social work competencies by department and hospital standardsCommunicates telephonically and electronically with the outpatient providers in an effort to enhance the continuum of care;Assumes responsibility for NL EMMC required continued education and own professional growthPerforms other duties as assigned or requiredYou'll be rewarded and recognized for your performance in an environment that will challenge you and give you clear direction on what it takes to succeed in your role as well as provide development for other roles you may be interested in.Required Qualification:Bachelors degreeLicensed by the state of Maine, LSWPay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The salary for this role will range from $49,700 to $88,800 annually based on full-time employment.
