As a Physical Therapist, you will work collaboratively with an interdisciplinary healthcare team to provide individualized treatment plans and high-quality patient care. Core Responsibilities Patient Evaluation Assess patients' functional status, strength, mobility, balance, and physical limitations. Develop individualized treatment plans tailored to each patient's rehabilitation goals and needs. Therapeutic Interventions Provide skilled physical therapy treatments and interventions designed to improve movement, function, and independence. Monitor patient progress and modify treatment plans as necessary to achieve optimal outcomes. Education Educate patients, families, and caregivers on prescribed exercises, adaptive techniques, safety precautions, and the proper use of assistive devices. Encourage patient participation in the rehabilitation process to promote recovery and long-term success. Documentation Maintain accurate and timely documentation of evaluations, treatment plans, patient progress, and outcomes. Ensure compliance with facility policies, professional standards, and regulatory requirements. Team Collaboration Collaborate with physicians, nurses, case managers, and other healthcare professionals to coordinate patient care and support successful transitions of care. Participate in interdisciplinary treatment planning and discharge recommendations.
