Home Health RN Case Manager – Fort Worth / Tarrant County, TXWe are seeking an experienced Home Health RN Case Manager for a field-based role serving Fort Worth, TX and the surrounding Tarrant County area. This position is best suited for a clinically strong Registered Nurse with recent home health case management experience, strong assessment skills, solid documentation habits, and the ability to manage an assigned patient caseload independently in the field.Position SnapshotRole: Home Health RN Case Manager / Field RNLocation: Fort Worth, TX and surrounding Tarrant County areaSetting: Medicare-certified home healthSchedule: Full-timeExperience Required: Recent Home Health RN Case Management experienceKey ResponsibilitiesManage an assigned home health patient caseload in the field.Perform skilled nursing visits, assessments, evaluations, recertifications, resumptions of care, and discharges.Develop, update, and coordinate individualized plans of care in collaboration with physicians, patients, families, and the interdisciplinary care team.Complete accurate, timely clinical documentation, including OASIS and visit notes.Monitor patient progress, identify changes in condition, and communicate updates appropriately.Provide patient and caregiver education related to disease management, medications, safety, and plan-of-care goals.Coordinate care with therapy, social work, aides, physicians, and office/clinical leadership.Maintain compliance with home health regulatory, documentation, and quality standards.Required QualificationsActive Texas RN license in good standing.Recent Home Health RN Case Management experience required.Strong understanding of Medicare home health, OASIS, plan-of-care management, and field-based nursing.Ability to work independently while maintaining strong communication with the clinical team.Strong clinical judgment, organization, documentation, and time-management skills.Valid driver's license, reliable transportation, and ability to travel locally throughout Fort Worth / Tarrant County.EMR experience preferred.The best fit is a dependable, field-ready Home Health RN Case Manager who knows how to manage a caseload, complete documentation accurately and on time, communicate well with patients and the care team, and deliver high-quality skilled nursing care in the home setting.
