Job Description
Licensed Practical Nurse (LPN)

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our team. The successful candidate will play a critical role in providing high-quality care to our patients.

Key Responsibilities

Provide nursing care to patients, including assessing their needs and developing individual care plans.
Administer medications and treatments as prescribed by the healthcare team.
Assist patients with daily activities and personal care to ensure comfort and safety.
Monitor and document patient progress, reporting any changes to the nursing staff.
Collaborate with other healthcare professionals to enhance patient care.
Participate in patient education and support healthcare initiatives.

Qualifications

Current LPN licensure is required.
IV Certification is required.
Experience in geriatric care or working with patients with chronic conditions is preferred.
Strong communication and interpersonal skills.
Ability to work effectively in a team-oriented environment.
Flexibility to work weekends and holidays every other rotation.

Shifts

Shifts available in both 8-hour and 12-hour formats.
Location-specific orientation will be provided, requiring 1-2 shifts.

Patient Demographics

The ideal candidate will be comfortable working with a diverse patient population, including individuals with:

Dementia
Diabetes
Hypertension
Chronic kidney disease
Mobility challenges and ADL assistance needs

Additional Information

No Charge Differential at participating facilities.
Backouts and quits with less than 14 days' notice will incur a fee.
