Responsibilities Assess patient needs and develop individualized rehabilitation plans. Deliver physical therapy interventions, monitor progress, and adjust treatment plans. Educate patients and caregivers on exercises and self‑care techniques. Collaborate with interdisciplinary team members and coordinate referrals. Maintain accurate documentation of patient assessments, progress notes, and treatment plans. Qualifications Valid Physical Therapy license in the state(s) of practice, or proof of license and eligibility. Master's degree in Physical Therapy, or a Physical Therapy related master's plus a bachelor's in Physical Therapy, or bachelor's in Physical Therapy with 5 years progressive experience.
