Primary Care Physician
Join a physician-led primary care organization built around outcomesnot volume. This role offers manageable patient panels, strong care team support, and the ability to practice medicine the way it was intended: with time, accountability, and purpose.

Your Role:
- Provide comprehensive primary care to older adult patients
- Maintain full accountability for clinical outcomes and patient experience
- Serve as a health coach, guiding lifestyle and long-term health improvement
- Coordinate specialty and hospital care while staying actively involved
- Lead and collaborate with an interdisciplinary care team
- Supervise and remain accountable for Advanced Practice Providers (APPs)
- Partner with medical and operational leadership to meet quality and cost goals
Performance Focus (Supported & Measured):
- Hospital admissions per thousand
- Appointment utilization (1821 slots per day)
- Patient experience (CG-CAHPS)
- Closure of clinical care gaps
- Medical cost management
Qualifications:
- MD or DO required
- Board Eligible or Board Certified in Internal Medicine, Family Medicine, or Geriatrics
- Active or eligible state medical license
- Active DEA
- Commitment to value-based, team-oriented care
Compensation & Benefits:
Salary: $250,000 $306,714 (based on experience)
