Title: Senior Social Worker

Location: Flagstaff, AZ 86001
Duration: 12-Month Contract (Extension or Direct Hire Possible)
Schedule: Monday-Friday, 40 Hours Per Week
Pay Rate: Up to $33.65/hr on W2

Seeking a Senior Social Worker. This role will oversee social service programs, provide case management, coordinate care planning and discharge services, supervise social services staff, and advocate for veterans and their families.

Required Qualifications:

• Master's Degree in Social Work (MSW).
• Active Arizona Social Work License (or ability to obtain upon placement).
• Experience with case management, care planning, resident assessments, and discharge planning.
• Knowledge of CMS regulations, PASRR, MDS systems, and long-term care operations.
• Strong leadership, communication, and organizational skills.

Preferred Qualifications:

• Supervisory experience in a healthcare setting.
• Long-term care, skilled nursing, or geriatric social work experience.
• Experience working with veterans and community resources.
