Job Overview

The Primary Care Physician serves as the leading physician within a multidisciplinary PACT team or acts as a float or gap provider, delivering comprehensive and collaborative acute and ongoing care to a designated group of Veterans. This role may also extend to patients outside the assigned panel when necessary for Veteran care.
A relocation or recruitment incentive may be offered to exceptionally qualified candidates.
This position qualifies for the Education Debt Reduction Program (EDRP).

The responsibilities of the Primary Care Staff Physician encompass, but are not limited to:

• Providing continuous care for one or more assigned panels of Veterans.

• Utilizing available resources, such as registries, to effectively identify and intervene with individual patients and groups.

• Ensuring that patients within the assigned panel receive appropriate evaluations and access to care.

• Operating within the full scope of the team member's clinical privileges, credentials, and approved documentation of competency.

• Engaging in team performance improvement initiatives to enhance efficiency and patient care delivery.

• Implementing management processes for primary care operations as needed.

• Facilitating safe transitions for patients between the PACT's care site and other healthcare environments through effective communication.

• Providing health education and coaching on wellness, disease prevention, chronic care management, and self-management skills to patients and their support persons, based on the PACT member's expertise.

• Encouraging patient engagement in their healthcare and involving personal support persons in the process, while valuing their input regarding VA care.

• Maintaining respectful, effective, and timely communication with all team members, including patients and their support persons, to share significant clinical information.

• Delivering healthcare services in accordance with the PCP's licensure and clinical privileges.

• Ensuring that the patient's care plan includes medically necessary recommendations.

• Providing or arranging clinically indicated healthcare services for patients within the PACT.

• Guaranteeing same-day access for both face-to-face and telephone consultations during regular clinic hours.

• Collaborating with PACT staff to create personalized health plans that address care management and coordination based on patient needs.

• Leading the implementation of the PACT model and clinical practice guidelines to enhance patient care outcomes and foster high-performing teams.

• Completing Women's Health Training certification and delivering Women's Health services.

• Providing gender-specific care in line with VHA Directive Health Care Services for Women Veterans, including routine pap smears, breast health, and management of menopause and osteoporosis.

• Participating in the education and supervision of medical students and residents, ensuring compliance with GME requirements while promoting a supportive environment for education and research.

• Responding promptly to patient-related communications from other clinicians, regardless of the communication method, to facilitate coordinated patient care.

• Ensuring that patients are informed of normal diagnostic results within 14 days and abnormal results requiring action within a clinically appropriate timeframe, not exceeding 7 days.

• Performing additional duties as assigned by the Chief Medical Officer.
