RN - Case Manager

Care ManagerThe primary goal of the Care Manager is to optimize members' health care and enhance the delivery of care experience. This will lead to expected cost savings due to improved quality of care. The role involves engagement and understanding of the member's needs, environment, providers, support system, and optimization of available services. The Care Manager is expected to assess and evaluate member's needs while being a creative, efficient, and resourceful problem solver. In collaboration with the member's care team, a plan of care with individualized goals and interventions will be developed, implemented, and outcomes evaluated. Computer literacy is required. Ability to proficiently read and interpret medical records, claims data, pharmacy, lab reports, and prescriptions is necessary.
