Registered Nurse Case Manager

Bethesda Corporate; Home Health is seeking a Registered Nurse to organize, coordinate and direct services for patients in the agency. You will ensure quality care, supervise team members when needed, and maintain open communication with patients, families and physicians to promote positive outcomes. You will develop and update the plan of care, document in the EMR at point of care, complete physician summaries at SOC/Recert/Transfer/ROC, and maintain accurate visit schedules while participating #J-18808-Ljbffr
