Must be licensed in the State of Missouri as a Registered Nurse, have excellent communication and computer skills as well as reliable transportation.Job Responsibilities:Assist the men with scheduling and coordination of health care specialistsAccompany Jesuits to physician or specialist appointments as neededReport changes in condition and relevant health updates to the nurse on duty, Care Coordinator Nurse, and Jesuit leadershipManage admissions and readmission, including communication of condition, treatments, and medicationsEnsure care plans are accurately and promptly communicated to the Care Coordinator Nurse and/or floor nurse on dutyAssume full responsibility for assigned casesComplete documentation, charting, and planning in accordance with prescribed proceduresDelmar Gardens is a family-owned, St. Louis-based company that has been serving the community for 60 years.
