Role: Discharge Planning Nurse Responsibilities: Assess, develop, implement, and evaluate patients' discharge plans in collaboration with an interdisciplinary team to ensure smooth transition to the next level of care. Ensure compliance with discharge planning regulations and identify gaps in inpatient and observation stays. Assess high-risk patients for readmission, identify root causes, and collaborate on discharge strategies. Participate in quality improvement initiatives and may handle utilization management, ED case management, and act as a liaison with post-acute providers. Apply evidence-based practices, adhere to professional nursing standards, and follow the ANA Code of Ethics. Maintain safety awareness and perform additional duties as assigned. Qualifications & Skills: Graduate of an accredited nursing program, with at least three years of acute care experience. Licenses: Valid RN license in applicable states, including Illinois, Missouri, Oklahoma, or Wisconsin, with appropriate certifications. Physical Requirements: Ability to perform frequent lifting, sitting, standing, walking, and other physical activities, along with good vision and hearing capabilities. Work Schedule: Day shift, 8:00 am - 4:30 pm, with a 40-hour workweek. #J-18808-Ljbffr Health Here
